Dosierung (الجرعة)

- ITP: 25–75 mg 1×/d p.o.

Indikationen (الاستطبابات)

- Immunthrombozytopenie (ITP) nach Versagen von Steroiden/IVIg
- Schwere aplastische Anämie

Kontraindikationen (موانع الاستخدام)

- Schwere Leberinsuffizienz (Child-Pugh C)
- Thromboembolie-Anamnese (relatives Risiko)

Merksatz: (جملة للحفظ) TPO-RA bei ITP = 2./3. Linie nach Steroiden. Eltrombopag oral, Romiplostim s.c.

Nebenwirkungen (الآثار الجانبية)

- Hepatotoxizität (Leberwerte kontrollieren!)

Interaktionen (التفاعلات)

- Polyvalente Kationen (Ca, Mg, Al, Fe, Zn in Antazida/Nahrung): Chelat-Bildung → Resorption ↓ → Einnahme nüchtern, 4 h Abstand zu Antazida
- Rosuvastatin: Spiegel ↑ (OATP1B-Hemmung)
- Ciclosporin: Spiegel ↑

Wirkmechanismus (آلية التأثير)

Oraler, nicht-peptidischer TPO-Rezeptor-Agonist → bindet transmembranäre Domäne des c-Mpl-Rezeptors → JAK2/STAT-Aktivierung → Megakaryopoese ↑ → Thrombozytenproduktion ↑.

Monitoring (المراقبة)

- Thrombozyten wöchentlich (Titration)
- Leberwerte (ALT, AST, Bilirubin) monatlich
- Augenuntersuchung (Katarakt!) [VERIFY]
