M. Basedow, TSH < 0,01, fT3 ↑↑ → Thiamazol 20 mg/d + Propranolol. 3 Wo später: Fieber 39°, Halsschmerzen → BB: Leukozyten 800, Neutrophile 50 → Agranulozytose → stationär!
- Häufig: Hautausschlag, Arthralgie, GI-Beschwerden
- Gefährlich: Agranulozytose (0,1–0,5%! — akut, idiosynkratisch!)
- Prüfungsrelevant: Fieber + Halsschmerzen → SOFORT BB → Agranulozytose?
- β-Blocker: synergistisch bei thyreotoxischer Krise
- Antikoagulantien: Effekt variabel (Schilddrüsenstatus beeinflusst Gerinnung)
Hemmt Thyreoperoxidase → Jodorganifizierung und Kopplung ↓ → T3/T4-Synthese ↓. Wirkt NICHT auf bereits gespeichertes Hormon (Latenz 1–3 Wochen!).
- BB: initial regelmäßig + bei jedem Fieber/Halsschmerzen!
