Recognition of Egyptian Medical Studies
Egyptian Medical Studies typically last six years (MBBCh) and, since the 2018 reform, are concluded with a two-year mandatory Imtiyaz (الامتياز) — previously it was one year. Only after completing the Imtiyaz do you receive full Approbation from the Egyptian Medical Association (نقابة الأطباء) and are permitted to practice independently.
For German Approbation, the responsible Approbation authority of the Federal State (e.g., LPA Hesse, LAGeSo Berlin, District Government NRW) checks whether your education is considered equivalent to the German one. In almost all cases, the decision is: “significant differences” — and you must take the Kenntnisprüfung (KP). Since the Bundestag resolution of March 26, 2026, the KP for third-country nationals is no longer treated as an exception but as the standard case. An Equivalency Examination with the goal of direct Approbation without KP is very rare in practice for Egyptian diplomas.
Required Documents
For the application to the Approbation authority, you generally need:

Consular Legalization — the Egyptian way (no Apostille!)
This is the most important difference from many other countries: Egypt is not a member of the Hague Apostille Convention. Therefore, you cannot apply for an Apostille — and authorities that only accept an Apostille must refer you to the Egyptian Procedure. Instead, your documents require full consular legalization. The order:
- University / Issuing Authority certifies the original document (e.g., Dean’s Office of the Faculty of Medicine, نقابة الأطباء for the license).
- Ministry of Higher Education (وزارة التعليم العالي) in Cairo over-certifies the university documents.
- Egyptian Ministry of Foreign Affairs (وزارة الخارجية المصرية) in Cairo then legalizes.
- German Embassy Cairo checks and finally legalizes.
Realistically, plan two to six months for this process — depending on whether you are on-site or working with a notarized power of attorney. The German Embassy Cairo does not automatically accept all documents; in case of doubt, an additional authenticity check is conducted via the embassy, which can take weeks to months.
Tip: Ask the Dean’s Office to issue an English Verification Letter template — many Approbation authorities accept this as a supplement to legalization.
§16d AufenthG — Visa for Recognition
If you are still in Egypt and coming to Germany for recognition, the §16d Visa (residence for the recognition of foreign professional qualifications) is the standard path:
- Duration: up to 18 months for recognition measures (language Course, FSP, KP Preparation)
- Extension by up to 12 months for job searching after successful recognition
- Part-time employment up to 20 hours/week permitted — many Egyptian colleagues work during this time as nursing assistants, interns, or already as Resident Doctors with a Berufserlaubnis
- Prerequisite: proven intention for recognition, secured financing (blocked account), usually A2/B1 German upon application

Kenntnisprüfung — what to expect
The KP is an oral-practical examination of approximately 60–90 minutes before a commission of at least three specialists. Four main areas are examined: Internal Medicine and Surgery are mandatory, plus two elective subjects (often Emergency Medicine, Pharmacology, Psychiatry, or Radiology).
Procedure: Examine Patient, read files, present case to the commission, Differential Diagnosis, Therapy plan — all in German. The failure rate is high in some Federal States (over 30%). The examination can be repeated twice — after the third Attempt, Approbation in Germany is definitively no longer possible.
Egyptian Doctors often have very good clinical prior knowledge from Imtiyaz and working in busy hospitals. Typical gaps lie in German guidelines (DGIM, AWMF), legal Topics (Informed Consent, living will, IfSG), and structured case presentation in the German style.
Fachsprachprüfung (FSP) — B2 + C1 Medical
Before or parallel to the KP, you need the Fachsprachprüfung from the responsible Medical Association. Three parts, 20 minutes each:
- Doctor-Patient Conversation (Medical History)
- Written Medical Report
- Doctor-Doctor Handover (case handover)
Prerequisite: B2 General German (telc, Goethe, ÖSD). The examination is at C1 Medical level.
