What is the Kenntnisprüfung? — Definition and Legal Basis

The Kenntnisprüfung (KP) is a state examination according to § 3 Para. 3 of the Federal Medical Regulations (BÄO). It is taken by international doctors whose medical studies have been classified by the responsible authority as not equivalent to German medical studies. The KP is therefore the central hurdle on the path to Approbation — the permanent medical Berufserlaubnis in Germany.

The exam is organized by the Landesprüfungsämtern of the respective Federal States and administered by an Examination Committee consisting of at least two medical members. It has an oral-practical structure and tests whether your medical knowledge meets the requirements of the third section of the German medical examination (M3).

⚠️ Achtung: Sie haben maximal 3 Versuche. Nach dem dritten Fehlversuch ist die Approbation über diesen Weg dauerhaft ausgeschlossen. Im Gegensatz dazu kann die Fachsprachprüfung (FSP) unbegrenzt wiederholt werden. Jeder Versuch der KP zählt — auch bei einem Bundeslandwechsel.

Who Needs to Take the Kenntnisprüfung?

The Kenntnisprüfung applies to doctors who:

  • Obtained their medical degree outside the EU/EEA/Switzerland (e.g., Syria, Iraq, Egypt, Jordan, Morocco, Tunisia, Iran, Pakistan, Ukraine, Russia)
  • Have received a Deficiency Notice from the responsible Recognition Authority, which identifies significant differences from German medical studies
  • Have opted for the Kenntnisprüfung route instead of the Equivalency Examination (Approbationsprüfung)

Not affected: Doctors with degrees from EU/EEA countries generally receive automatic recognition in accordance with EU Directive 2005/36/EC. The Kenntnisprüfung is not required for them.

Tip: Before the KP, you will first need a Berufserlaubnis (temporary work permit as a doctor) to gain clinical experience in Germany during the preparation period. This is often applied for in parallel with the Recognition Procedure.

Exam Subjects and Content of the Kenntnisprüfung

The Kenntnisprüfung tests knowledge at the level of the third section of the medical examination (M3). The subjects are divided into mandatory main subjects, a variable third subject, and interdisciplinary areas:

Mandatory Subject 1: Internal Medicine

Internal Medicine is the focus of every Kenntnisprüfung. The relevant sub-areas and the matching Kennti courses:

Variable Third Subject (according to Defizitbescheid)

Exam Format: 6 Sections in Detail

The Kenntnisprüfung simulates a real clinical situation. You will receive a Patient (simulation patient or real finding) and go through six sections:

  1. Medical History (approx. 15 min.) — Structured patient conversation according to SAMPLE or OPQRST scheme. The examiners assess whether you ask targeted questions, listen actively, and systematically record medically relevant information.
  2. Physical Examination (approx. 10–15 min.) — Systematic examination of the patient. You must explain your steps, recognize pathological findings, and document them.
  3. Diagnosis — Formulate a Suspected Diagnosis and up to 3 Differential Diagnoses. Justify your decision with the findings obtained.
  4. Diagnostik — Labor anfordern und interpretieren, Bildgebung wählen (Röntgen, CT, MRT, Sono), EKG-Befunde lesen. Radiologie-Kenntnisse sind hier entscheidend.
  5. Therapy Plan — Acute therapy and long-term treatment according to current guidelines. Name medications with Dosage, Contraindications, and Side Effects.
  6. Case Presentation — Write a Medical Report (Konjunktiv I, Passive Voice) or oral Handover according to the ISBAR scheme to the “Senior Physician”.

Assessment: The exam is assessed with “passed” or “failed” — no grades are given. The Examination Committee assesses medical expertise, clinical thinking, communication skills, and the structuring of the medical procedure.

Brauche ich die FSP vor der KP?

Ja, in den meisten Bundesländern ist die bestandene Fachsprachprüfung Voraussetzung für die Zulassung zur KP. Wenige Ausnahmen erlauben eine parallele Vorbereitung.

Kann ich das Bundesland für die Prüfung wechseln?

Grundsätzlich sind Sie an das Bundesland Ihres Hauptwohnsitzes gebunden. Ein Wechsel ist unter bestimmten Bedingungen möglich (z. B. Umzug), aber die Fehlversuche werden bundesweit gezählt und nicht zurückgesetzt.
